We see patients by appointment only.  To make an appointment please call (502) 895-6559 during office hours.

Our office hours are:

  • Monday, Tuesday and Thursday: 8:30 a.m. to 4:00 p.m.
  • Wednesday: 8:30 a.m. to 3:00 p.m.
  • Friday: 8:30 a.m. to 2:30 p.m.
